Re-run the check whenever the itinerary changes. A deeper site, different vessel, new transit country, additional breathing gas or newly disclosed health detail can change the question the insurer must answer. Confirmation for one trip should not be assumed to apply to another.

Do not ask, “Does it cover scuba?” Ask, “Does it cover this itinerary?”

Give the insurer the destination, operator or vessel, maximum planned depth, certification, supervision, breathing gas, dive frequency and every specialist activity. Ask for the exact wording that supports the answer and keep it with the Certificate of Insurance.
